New Patient Tips for Your First Veterinary Visit with Us
Before you bring your pet in for their first exam, here are a few simple steps you can take to help ensure a productive visit:
- Complete the online new patient form. Alternatively, you can print, complete, and bring it in with you on exam day if preferred.
- Bring your pet’s up-to-date health and medical documents – including history of vaccinations/vaccination schedule and preventative care records. If you are unable to obtain your pet’s records prior to visiting, we’re happy to procure them on your behalf. Simply fill out the information in the new patient form. If you have your records electronically, you can email to them to your local Vitality Veterinary Services animal hospital.
- Bring a stool sample – If this is your pet’s annual wellness visit and/or if your pet is suffering from GI issues. If your pet is having urinary issues, we also recommend obtaining a urine sample, if possible. While these tasks may not be pleasant or easy, they provide us with valuable information about your pet’s current state of health and can help to improve your pet’s first visit.
